Grievance Policy

A Giving Tree Adoptions is committed to providing quality services by treating everyone involved in the adoption process with fairness and professionalism. The purpose of this form is to ensure that each person (biological parents, adopting families, and children involved in the adoption process) is given the opportunity to express a problem or grievance related to the quality of services. 

If you feel that your needs have not been nor are they being met, for whatever reason, please talk to your caseworker or program coordinator.  There could be a number of reasons for feeling this way and we want to make sure everyone is on the same proverbial page.

After speaking with your caseworker, if you do not feel resolution to your concerns, please complete and forward the Formal Complaint/Grievance Form to our Executive Director, Elizabeth.  Once notified in writing, Elizabeth will begin to investigate the matter within two (2) business days.  Elizabeth will report the outcome of the complaint investigation to you within ten (10) business days after the complaint is received.  If it has not been possible to gather the necessary information that would lead to a resolution within ten days, you will be notified and given a new date, up to 30 calendar days, by which a resolution or determination will be made.  Elizabeth will share with all parties involved a written and objective decision regarding the concern, and how it might best be resolved.

If you are still feeling unsatisfied with the results, you may contact a member of the AGTA Board of Directors to discuss the matter.  Board members will review the matter and respond to you within ten (10) business days of the receipt of an updated Formal Complaint/Grievance Form to a member of the AGTA Board, information on Board members can be obtained from Administrative Assistant.  The decisions and recommendations of AGTA Board members will be final.

Any retaliation against the person making the complaint is strictly prohibited.  The Executive Director conducting the investigation shall monitor to ensure that there is no retaliation against the child or the person who files a grievance.

This form is to be completed if you wish to make or file a grievance or complaint. You may also ask someone else who is acting with your knowledge and consent to write or express the grievance. (You may file this report anonymously, without your name, however it may make it more difficult for AGTA to address the matter.)
